HEALTHY REMINDERS
Annual routine physical and well child exam for yourself or your covered dependents are covered at 100%, in addition to immunizations, annual labs, routine mammograms and colonoscopies, as recommended by the Affordable Care Act (ACA).
Refer to www.healthcare.gov/coverage/preventive-care-benefits for a complete listing of covered preventive services.
Need a DOT physical? DOT physicals for eligible employees are covered at 100% provided the routine physical benefit hasn’t been previously been used.
TIPS FROM YOUR TRUST
Please complete a new enrollment form if you have:
A new dependent to enroll
A change in marital status
If you have moved and have a new address or contact information
Please contact the Trust Office if there has been a death of an active Participant or enrolled Dependent
